Evaluation of a Safety Program

A systematic approach to safety evaluation can be a valuable tool to the safety official and to the engineering manager. The effective use of this tool will reduce the frequency of mishaps, and when mishaps do occur (as in the Apollo 13 mission), it will assure that a maximum number of alternatives are available to minimize the danger to personnel.
